Finally, milk thistle's anti-inflammatory and immunomodulatory properties offer a possible therapy approach for oral mucositis, a frequent side effect of cancer medications [4].

Preclinical and Clinical Trials of Milk Thistle

Dental caries and periodontal disease: By influencing NF-κB activation and translocation and lowering proinflammatory cytokines, silymarin has anti-inflammatory properties. In vitro, silymarin improves gingival fibroblasts' cell viability and, in combination with resveratrol, dramatically reduces the release of IL-6 and IL-8. The antibacterial properties of silibinin and silymarin against cariogenic microorganisms point to their potential for reducing tooth caries.

Oral Candidiasis: In vitro research demonstrates that silymarin suppresses the development of Candida albicans, downregulates virulence genes, breaks down biofilms, and prevents the release of hydrolases. A silybin-containing substance increases membrane permeability by promoting depolarization and decreasing the fluidity of fungal cell membranes. Extract from silymarin prevents C. albicans from damaging its membranes and forming biofilms.

Oral Lichen Planus: Systemic silymarin therapy significantly reduces lesion size and discomfort in people with Hepatitis C, according to clinical studies. There is a decrease in serum TNF-α levels. A small body of research indicates that milk thistle extract has steroid-like, wound-healing, T-cell-suppressive, and antioxidant properties [4].

Oral Cancer: Without causing harm, silymarin prevents the development of oral cancer cells, triggers apoptosis, and slows the formation of tumors in mice. Apoptosis is linked to caspase 8 and death receptor 5. As a risk-free and efficient chemopreventive treatment for oral cancer, milk thistle exhibits potential.

Chemotherapy-induced oral mucositis: Silymarin efficiently lowers pain and inflammation, boosts antioxidant capacity, and hastens the healing process. At some dosages of silymarin, protective effects against radiation-induced oral mucositis have been demonstrated.

Dosage, Administration Method, and Safety: In mice and rats used in experimental models, silymarin dosages do not cause significant toxicity. There are several teratogenic and reproductive issues that need to be looked at more. Oral silymarin dosages ranging from 175 to 420 mg/day for varied durations in healthy persons do not appear to have any negative side effects in humans. Treatments tailored to a particular disease that use 140 mg of silymarin two to three times a day are efficient, well-tolerated, and have little side effects. Administering intravenous dosages might cause mild, temporary adverse effects [4].

Clinical Trials on Oral Disorders: The least effective oral dosage of silymarin (140 mg three times a day) is used in limited clinical trials for oral disorders. Oral lichen planus and radiation-induced oral mucositis in Hepatitis C patients have been found to respond well, with minimal side effects [4].
